1. Duly-filled in application form in duplicate.
2. Valid passport supported by valid visa or any other Immigration document regularizing stay in Canada.
3. Identity card or any other Original Document establishing applicant's identity as an Ex-Service man.
4. Applicant to come in person.
   
5. Time taken: Issued the same day.
