Duplicate in Lieu of Lost or Damaged Passports (will be done at the Consulate)

Requirements

1. Fees C$ 255.00 payable to the Consulate General of India, Toronto by
   Bank draft/Money Order/Certified Cheque..
2. Passport Application Form (5 copies) .
3. Additional details form (5 copies) .
4. Eight Photographs  Black & White or Colour  against a light background showing full frontal view of the face (size 35mm x 35mm) .
5. Authority to disclose (please click here for the form) .
6. Photocopy of lost/damaged passport (if available) .
7. Police Report
8. If Landed Immigrant -
1. Copy of Record of Landing .
2. Original (or if lost, Certified copy) to be shown to Consular Officer.
9. If not a Landed Immigrant -

1. Certificate from Immigration Canada on category/class under which application for landing is pending .
2. Proof of withdrawal/rejection of refugee claim (Original and photocopy) .
3. Any other immigration document. (Original & copy) .
4. Signed statement on how you left India and came to Canada.
5. Other relevant documents. - Will be intimated by the Consular Officer depending on the case history.
9. Local photo identity card (attach a copy and show original) .
10. All applicants are required to apply in person .

Please note that issuance of duplicate passport depends on various clearances from competent authorities and there is no specific time frame for issuance of such passports.
