Requirements

1. Duly filled in Miscellaneous Services application form .
2. Current passport with copy of the relevant pages
3. Original Landing Paper, PR Card if applicant is a Permanent Resident,  Immigration Document, with a photocopy
4. Document in support of the change required. (In case of change in date/place of birth, a Court Order issued by a First Class Magistrate from India is required)
5. Marriage/Divorce Certificates issued in India must be attested by Consular Section, Ministry of External Affairs, Patiala House, New Delhi .
6. Marriage/Divorce Certificates issued in Canada must be attested by Ministry of Government Services of Ontario, 77 Grenville St. 9th Floor, Toronto , Tel: 416-325-8416 in case of residents of Ontario .
7. Fees - C$ 20/- payable to Consulate General of India, Toronto by Bank draft/Money Order/Certified Cheque.
