Change of Address in Passport

(Change in address, inclusion of spouse's name, date/place of birth, etc)

Requirements

  1. Fill in online passport registration form at  http://passport.gov.in/nri/ this will create a PDF file.
  2. Take a print out of the online application form (PDF file) , fill in its page 2 & 3. Please ensure that specimen signatures are affixed,in black ink, at four spaces provided and a good quality photograph (of size 2inch X 2inch) is fixed on its first page).
  3. ou need to apply for reissue of Passport. A new passport will be issued.
  4. Current passport with copy of the relevant pages
  5. Original Landing Paper, PR Card if applicant is a Permanent Resident, Immigration Document, with a photocopy
  6. Document in support of the change required. (In case of change in date/place of birth, a Court Order issued by a First Class Magistrate from India is required)
  7. Marriage/Divorce Certificates issued in India must be attested by Consular Section, Ministry of External Affairs, Patiala House, New Delhi .
  8. Marriage/Divorce Certificates issued in Canada must be attested by Ministry of Government Services of Ontario, 77 Grenville St. 9th Floor, Toronto , Tel: 416-325-8416 in case of residents of Ontario .
  9. Fee CAD 102.00 (Fee $ 100/- + $2 Consular surcharge -for a 36-page booklet) + BLS C$ 7.40 including tax

Time taken: 3-4 weeks.

Police Clearance Certificate

Application for Police Clearance Certificate from India (should be mailed to BLS Inc, Unit 201, 10 Gillingham Drive, Brampton, Ontario L6X 5A5.


 Requirements

  1. Appropriate application form duly filled in.
  2. Current Indian passport with valid immigration papers or Canadian passport in original and photocopy.
  3. Fees: C$26/- + $2 Consular surcharge + C$ 7.40 as BLS charges with tax.

Processing Time: Issue of Police Clearance Certificate requires references to authorities in India and elsewhere. Consulate can issue PCC only on receipt of clear police report from authorities in India which may take several months. The Consulate would communicate to the applicant once the report is received and PCC is ready to be issued.

 

 

NOC for Minor's Passport

This service is available at the Consulate General of India, 365, Bloor Street East, 7th Floor, Toronto ON M4W 3L4

Requirements

  1. Document to be attested in original with one photocopy.
  2. Valid Passport and PR/Study Permit/Work Permit, as applicable, with a photocopy
  3. Fee as applicable, payable to the Consulate General of India, Toronto by
    means of a bank draft/Money Order/Certified cheque only.
  4. Applications can also be sent either by mail or in person.
Time taken : Documents are accepted in the morning from 0900 to 1130 hrs and delivered on the same day between 1500-1600 hrs
  Mail-in application takes 3 working days upon receipt of documents

Power of Attorney/other civil documents:
The Power of Attorney (POA) or other civil documents have to be submitted to the Consulate in original along with a photocopy of a recent photograph of the applicant and two witnesses. The applicant is required to affix the photograph, at the end of the subject matter on the documents to be attested, followed by specimen signature below the photograph and signatures, with name and address of two witnesses. Please ensure that the applicant signs on the same page as the main content of the document. The applicant is required to come in person and sign in the presence of the Consular Officer at the counter. The notarized POA is required to be authenticated by the Ministry of Government Services, Ontario Government (address: 700 University Ave., 6th Floor, Toronto) or the concerned Provincial authorities before submitting it to the Consulate.

$32 per person's signature:  for property related issue

$16 per person's signature:  for civil documents

Commercial Documents:
For attestation of commercial documents, the signature of Notary Public are required to be authenticated by the Ministry of Government Services, Ontario Government or by the designated authorities of the Province concerned.  A copy of the passport or any other identification document of the Director of the company is required.  An authority letter on behalf of the company is required in case it is submitted by any one else other than the Director of the company.

For attestation of Birth/Death/Marriage/Divorce/Educational Certificates etc. issued by the Canadian authorities, a photocopy of the document is required to be notarized by Public Notary and duly authenticated by Ministry of Government Services, Ontario Government or any concerned Provincial authority designated.

Will/Adoption documents:
The Will document is required to be notarized by the Notary Public and duly authenticated by Ministry of Government Services, Ontario Government or by the designated authorities of the Province concerned.

The Adoption document is required to be authenticated by the Ministry of Government Services of Ontario Government or any concerned Provincial authority designated.

Documents issued by Indian Authorities:
Birth, Death, Marriage and Educational Certificates etc. issued by the Indian authorities in India can also be attested by the Consulate provided the same have been authenticated by the designated officer of the State Government and validated by the Consular Section of the Ministry of External Affairs (MEA), Patiala House, New Delhi-110001 or any other agency approved by the Ministry of External Affairs. Driving License/Driving experience letter:
The Consulate does not authenticate Indian Driving license or experience letter.  Applicants are advised to obtain written authentication of their Indian driving license or driving experience directly from the Driving Licensing Authority of India.
 
   

No Objection Certificate (NOC) for Minor's Passport:
This NOC is required to be submitted along with the passport application in India or abroad. If one parent residing in India is applying for a minor's passport, the parent residing abroad should send an affidavit Form of 'no objection' attested at the Indian Mission/Consulate, which should be submitted along with minor's application;
or
If both parents are residing abroad and a legal guardian in India is applying for the minor's passport, both parents should send an authorization attested by the Indian Mission/Consulate abroad to the legal guardian authorized to apply for the passport along with the legal guardian's Affidavit with two witnesses known to the guardian.

Requirements:

Original Passport and copies of passport; first three and last two pages of the passport and any other page containing observation should be provided.

Proof of Residence:  Any one of  these i.e. copy of utility bill (phone/electricity bill) or driving licenses or lease deed (of residence) etc.

Copy of PR card (both sides) or Student Permit/Employment authorization letter  (for Indian passport holders only)

NOTE:  If the application is sent by mail then all photo copies should be duly notarized and authenticated by the Ministry of Government Services, Ontario Government or any concerned Provincial authority designated.

The Power of Attorney(POA) along with photograph must be notarized by a local Public Notary and authenticated by the Ministry of Government Services, Ontario or the Provincial authorities before submission by mail.

Certificate of Birth

This is not a Birth Certificate but a Certificate of the Date of Birth. Applicants are advised to check the acceptability of this Certificate with the authorities before obtaining this Certificate.

Requirements

  1. Application form duly complete .
  2. Current/expired Indian passport with valid immigration papers in original and a photocopy .
  3. Fees: C$ 26 + $2 Consular surcharge + BLS Int. Canada Inc. charges C$ 7.40 including tax.

 

Life Certificate

Life Certificate to be submitted at the Consulate
There is no fee charged for this service

Requirements

  1. Duly-filled in application form in duplicate.
  2. Valid passport supported by valid visa or any other Immigration document regularizing stay in Canada.
  3. Identity card or any other Original Document establishing applicant's identity as an Ex-Service man.
  4. Applicant to come in person.
  5. Time taken: Issued the same day.

 

Registration of Death

Registration of deaths (registered at the Consulate)

Death of all Indian nationals should be registered with the Consulate by submitting the prescribed application in duplicate together with the death certificate, burial/cremation certificate, passport of the deceased and fee of C$1.00 payable to Consulate General of India, Toronto by Bank draft/Money Order/Certified Cheque.

  6. Flight details.
  7. Fee for sealing of the body, etc C$ 195.00 payable to Consulate General of India, Toronto by Bank draft/Money Order/Certified Cheque.

 

Transportation of Ashes to India

Transportation of Ashes to India (Documentation to be submitted at the Consulate)

Requirements

  1. Registration of Death with the Consulate Form (for Indian nationals only).
  2. Death Certificate in original and a photocopy .
  3. Cremation certificate and a photocopy.
  4. Certificate of packing indicating name, age, date, time and cause of death, contents of package.
  5. Passport of the deceased and a photocopy.
  6. Name, address and passport particulars of the person taking the ashes and a photocopy of the passport.
  7. Fees: C$48.00 for Canadian nationals of Indian origin. Fees payable to Consulate General of India, Toronto by Bank draft/Money Order/Certified Cheque.
